    The Demarest Board of Education requires each new student, whether entering at preschool level or later, to provide documentation of a recent (within 365 days of school entry) physical examination and proof of immunization. There is a standard examination form that will be given to each student.  The physical exam should be conducted by a healthcare provider or advanced practice nurse chosen by the student's parents or guardian at the provider's facility.  Parents and guardians are encouraged to contact the health office if a student's medical condition changes.
     
    Students registering at Demarest Public Schools will be given the required medical documents during the registration process.  The parent immunization letter outlines the state required immunization and tuberculosis guidelines for students in New Jersey schools.  The physical and immunization record will be used to document the child's recent physical exam and record immunization history.
