In order to try out/participate on the DMS volleyball team, your child must have a current physical on file. The physical must be completed on the state mandated forms linked below. No other physical form will be accepted to play sports. To be current, they must have had a physical within a year to the first practice. Please be aware that if your child's physician is from out of state, they must be willing to complete a cardiac assessment module for educational purposes mandated by the NJ BOE. All the forms below must be completed to try out for the team. The athletic physicals form is the proper form to use for sports (this is the form used from now through high school). It can be done anytime--please fill out the history part of the form and bring it with the physical for your child's doctor to complete. The other forms are informational and the state requires signatures for the concussion and cardiac information. Please sign and return the signature pages of these no sooner than August.
